Main Purpose of Job

To provide professional Clinical Social Work services and Case Management to patients/residents and their families/significant others, guardians, and other surrogates of Western Maryland Hospital Center.

Per the Amended Directive and Order RegardingVaccination (MDH # 2023.04.18.03) issued by the Secretary of the MarylandDepartment of Health, effective April 18, 2023, staff in all hospitals andresidential facilities operated by the Maryland Department of Health or anylocal health department are required to show proof of COVID-19 vaccination.Staff includes, but is not limited to, regular and contractual employees,contractual staff, volunteers, and other state employees performing any dutiesat the facility. An individual may request an accommodation by providingappropriate and sufficient documentation for bona fide medical or religiousreasons.

POSITION DUTIES

Completes a comprehensive biopsychosocial-spiritualassessment through assessment of the patient/residents’, in collaborationwith families, guardians or other identified support persons. Including theidentification of patient/family/guardian goals, the internal/externalstrengths and barriers associated to reaching these goals. Development ofSocial Work interventions to support achieving goals.Identifies Assesses, facilitates, plans,evaluates, and coordinates healthcare services on behalf of the patient/residentthroughout the patients/resident stay and coordinates a safe discharge planwith the treatment team.Participates in the interdisciplinarytreatment team; completes and provides guidance to the utilization reviewprocess, care planning/discharge process and family conferences; reviews careplans with patients/residents and updates the patient/resident goals;conducts family tours and is a liaison for the facility and communitypartners.Make safety part of thejob; monitor improvement of processes and explore ways the work environment canbe safer. Understand what a sentinelrisk event is and how to report it. Uses Safety Alerts appropriately.Social Work Services provided in the nursinghome level of care “shall provide or make arrangements for services toidentify and meet the resident’s medically related physical, social, andbehavioral health needs.” COMAR 10.07.02.30Presents pertinentinformation influencing patients/residents’ response to disease process ortreatment goals and prevention of reoccurring illness/injury. Provides therapeutic counseling to patients/residents aspart of their treatment plan to identify barriers to achieve the highest level ofindependence.Maintains written records anddocumentation in accordance withFederal and State regulations and in accordance with facility policy.

Collaborates with thepatient/resident, family, guardian and/or identified support person, as wellas the Interdisciplinary Treatment Team to establish patient/residentcentered goals, and interventions. Collaborate with the patient/resident,family, guardian and/or identified support person to reduce barriers and tomaximize the patient/residents’ functional independence.

AssistsInterdisciplinary Treatment Team in identification and review of patient strengthsand areas needing support through a culturally competent lens. Assumesleadership in the development of patient/resident centered care plan goals andto monitor progress.

Reviewscare plan goals with the patient/family, as required.

Provides patient/family education.

Coordinatesdischarge planning with the patient/residents’, family, guardian, and/orother identified support persons, community agencies, stakeholders, and theInterdisciplinary Team. Complete a discharge assessment of needs anddevelop a plan to best meet patient/residents' need, and follow-up afterdischarge, as needed.

Facilitatesupportive relationships through WMHC and with community support agencies andstakeholders.

Thetreatment team is educated on these components of care and how they willaffect achievement of the treatment goals.

Maintains written records on allpatients: biopsychosocial-spiritual assessment; progress notes; care plans;discharge summaries. Support in obtaining patient/resident advanceddirectives, guardianship order, and update insurance information, as needed,and as directed by policies and/or management.

Expands knowledge base of community resourcesand improves professional skills through attendance at educational programsand professional meetings.

Attends mandatory education and maintains alllicense requirements.

Keeps abreast of changes in Medicare/Medicaidcoverage and requirements.

MINIMUM QUALIFICATIONS

Education: Determined by the Maryland State Board of Social Work Examiners under the licensing requirements for Social Workers.

Experience:Two years of experience rendering clinical social work services in a health care or treatment setting subsequent to the receipt of an approved Master’s degree in Social Work from an accredited college or university.

Note: Candidates may substitute U.S. Armed Forces military serviceexperience at a rank of Corporal/Petty Officer or higher as acommissioned officer in Social Work classifications or SocialWork specialty codes in the Social Science, Psychology, and Welfare field of work on a year-for-year basis for the requiredexperience.

LICENSES, REGISTRATIONS AND CERTIFICATIONS

1. Candidates must be licensed as a Certified Social Worker or Certified Social Worker, Clinical by the Maryland State Board of Social Work Examiners prior to appointment to a position in State service. Applicants who do not have a license may also apply pending receipt of the required license. When the license is obtained, submit a copy to the Maryland Department of HealthTesting Unit.

2. Candidates appointed to positions in this classification may be assigned duties which require the operation of an automobile. Employees assigned such duties will be required to possess an appropriate operator's license valid in the State of Maryland.
